Forms

There are two types of Web forms that can be used.  A Web based submission form and a PDF (portable document file).

The Web based submission form should be no larger than 525 pixels wide and conform to a college approved template.

PDF (portable document file) should be used for large, complex forms, or for forms that need to be signed and mailed.
